Community Inclusion

Individual Support Packages provide a range of supports to people with disability, their families and carers to achieve personal dreams.

Support may include:

  • Supporting the person in their home to enhance independence
  • Access to essential aids and equipment
  • Case management and program co-ordination
  • Social and recreational participation in the community
  • Spending time with friends and learning new skills in our Day Program, Futures or Community Education programs.

A planner works with the person with disability and their family and carers to explore informal, formal and specialised disability supports and develop a plan to meet the person’s individual needs.

Individual Support Packages include Support and Choice, Community Options and HomeFirst.
