Community Transport provides a door to door transport service to assist people to remain independent and living in their own homes. Community Transport is not an emergency transport service.
Transport is available for social and health related appointments. Transport may be in the Shepparton area, to regional centres and to Melbourne. The service is available for people who are unable to use regular transport due to disability, disadvantage or geographic isolation.
Service users are welcome to bring a family member or carer along with them for support.
Requests can be made from the person requiring transport, the person’s family and friends or from service providers such as GP’s and hospitals. People must be deemed eligible for the program prior to accessing our Community Transport services.
Services users are asked to contribute to the service. The amount will be negotiated at the time of the request.
This service is jointly funded by the Victorian and Australian Governments through the HACC Program. For further information about HACC Services visit the Home and Community Care website at http://www.health.vic.gov.au/hacc/
